How to request records:

You may request records from the clinic or hospital where you undertook medical care. Collecting your records from elsewhere may facilitate efficient, quality care.

For imaging (x-rays, CT, MRI, ultrasound), please request the radiologist report (words) and the images (pictures). Usually, images are provided on a CD, and the report may be burned onto the CD or provided on paper.

For pathology (biopsies and surgical specimens), blood work, microbiology, or other laboratory results the report may be available from the clinic but would also be available from the facility that performed the analysis, which may be local or distant. You may look in the clinic or hospital records to find the name of the facility used.
