Required Reading:

• At least one week prior to surgery, read the instructions before surgery.

• Read your surgery consent form before your day of surgery: General (incisional) consent (form “C-M”). Notify your surgeon if you have questions about the risks of surgery or the consent form.

• Adults should complete their Advance Directive forms.
